Medical Home Portal
The Medical Home Portal is a project of the Department of Pediatrics, University of Utah Health.
The Portal’s Mission is to assist and support professionals and families in working together (the Medical Home model) to care and advocate for children and youth with special health care needs by providing reliable and useful information about their conditions and caring for them and knowledge of valuable local and national services and resources.
